How to become a patient of our clinic?

To become a patient of the Health Clinic in Rumia, you must complete and sign the declaration of choosing a primary care physician. We also encourage you to choose a health visitor and community midwife (the choice of a community midwife is for women).

In order to submit a declaration, please go to registration at our Clinic.

The patient should have an identity document with a photo and PESEL number

  • ID card,
  • passport,
  • driving license.
  • children – school ID.

The declaration can also be submitted via the website office.gov.pl

Integrated Management System Policy

Taking into account the continuous improvement of the quality of services provided and the security of processed information, the implementation of the quality policy and the information security system "PANACEUM” Sp. z o. o developed, implemented
and intends to constantly improve the Integrated Management System in accordance with the international standards PN-EN ISO 9001:2015-10 and PN-ISO/IEC 27001:2014-12.

We are committed to meeting the requirements of legislation and legal provisions relating to our activities in such a way as to minimize the harmful effects on the natural environment and we want to constantly improve the quality of services.

We intend to implement the above tasks of the policy through the following goals:

  • meeting the expectations of our patients and other interested parties by continuously improving the quality of medical services provided,
  • improving the skills and qualifications of all employees in improving the quality of our services "PANACEUM" Sp. z o. o,
  • constant improvement of competitiveness by improving the compliant management system
    with the requirements of the PN-EN ISO 9001:2015-10 and PN-ISO/IEC 27001:2014-12 standards.
  • building a strong organization open to changes, innovations and challenges,
  • raising employees' awareness of information security,
  • protection of patient information,
  • providing patients with complete and consistent information about the services of our Organization,
  • protection of information ensuring the security of medical services provided,
  • implementation of business processes using modern and secure information technologies,
  • proper selection of suppliers and control of information processes entrusted to them,
  • conducting periodic risk analysis related to information security,
  • monitoring the effectiveness of the applied security measures and quick response to incidents,
  • ensuring compliance with legal requirements regarding information protection,
  • providing integral information for current operation and development,
  • introducing and monitoring the security of an information security management system,
  • analysis of process and business risks and opportunities, which reflects the context and competences of our Organization in the existing situation aimed at meeting the needs of clients, other parties interested in the management system and independent control bodies.

The Directorate provides the resources necessary to implement this policy and declares the full commitment of its own and all staff in maintaining the integrated management system and its continuous improvement in order to provide patients with care and better conditions.

All employees of "PANACEUM" Sp. z o. o. I make responsible for the implementation of this Policy in relation to their workplaces and oblige them to show initiative in the field of process and service improvement.

We ensure that the presented Integrated Management System Policy is understood and complied with by the employees of our "PANACEUM" Sp. z o. o. and cooperating persons and companies.
