How to become a patient of our clinic?

To become a patient of the Health Clinic in Rumia, you must complete and sign the declaration of choosing a primary care physician. We also encourage you to choose a health visitor and community midwife (the choice of a community midwife is for women).

In order to submit a declaration, please go to registration at our Clinic.

The patient should have an identity document with a photo and PESEL number

  • ID card,
  • passport,
  • driving license.
  • children – school ID.

The declaration can also be submitted via the website office.gov.pl

In the interest of patient safety, Panaceum Ltd performs monitoring of adverse events.

What is an adverse event.

Definition from the Act on Quality in Healthcare and Patient Safety (Journal of Laws 2023, item 1692):

Adverse event – an event occurring during or as a result of the provision or omission of healthcare, which causes or may cause a negative effect on the patient's health or life, in particular death, physical injury or mental disorder, illness, life threat, need for hospitalisation or its extension, as well as damage to the body or mental disorder of the foetus.

An event whose outcome is a predictable result of the correct provision of healthcare is not an adverse event.

Adverse events should not be confused with the natural progression of the disease or predictable complications of therapy.

Events can be reported by employees, patients, and their relatives or witnesses to such events.

Adverse events include:

  • misdiagnosis due to misinterpretation of laboratory and radiological results,
  • Incorrect prescription or administration of medication (wrong patient, wrong drug, wrong dose, wrong route or time of administration),
  • inappropriate timing of actions (e.g. referral to hospital too late), unjustified discontinuation of therapy,
  • bodily injury resulting from a procedure, medication administration, vaccination, or performance of another medical procedure, e.g. ear syringing, suture removal, etc.
  • Incorrect collection, preparation or storage of the sample for testing, loss of the sample for testing,
  • incorrect information provided to the patient (e.g. regarding medication, preparation for an examination, the date of the appointment, or what to do in the event of an infectious disease),
  • lack of current technical inspections or allowing individuals without the required permits to work
  • Accidents and injuries on the premises of the Panaceum Sp. z o.o. clinic.

The purposes of reporting adverse events:

  • Improving patient safety.
  • Implementation into practice of developed solutions that would have an impact on the elimination or reduction of adverse events.
  • Analysis of adverse events – determination of circumstances and causes.
  • Reporting adverse events is separate from the complaints register; it is not for stigmatisation but solely for initiating corrective actions.

Method of reporting:

  • You can report an adverse event that occurred at our clinic electronically: przychodnia@panaceum-rumia.pl, by completing the form below, or by using the paper form available at the clinic's reception, which can be placed in a dedicated box at reception.
  • The received reports are monitored and analysed by a dedicated adverse events team.
  • All information provided in the report is confidential and will only be accessible to authorised persons for the analysis of the adverse event.
  • The form is anonymous, but we encourage you to provide your name and email address. In case of any doubts, this will allow us to contact you and obtain additional information related to the reported adverse event..

Caution:

Every patient has the right to report adverse reactions to medicinal products. This includes both serious reactions and those that cause discomfort or concern. More information:
