How to become a patient of our clinic?

To become a patient of the Health Clinic in Rumia, you must complete and sign the declaration of choosing a primary care physician. We also encourage you to choose a health visitor and community midwife (the choice of a community midwife is for women).

In order to submit a declaration, please go to registration at our Clinic.

The patient should have an identity document with a photo and PESEL number

  • ID card,
  • passport,
  • driving license.
  • Children - school ID.

How to register?

Down Healthy Children Clinic please register by phone or in person after 9.00 a.m..
Possibility to register up to two weeks in advance.

Regulations for online registration to the clinic for healthy children

  • Please register only healthy CHILDREN (up to 0 to 6 years old):
  • for balance, vaccination and preventive examinations (measuring, weighing)
  • newborns without a PESEL number and declaration can only be registered in person or by phone
  • To your doctor or to the doctors whose names appear after selecting "ALL".
  • Please report your resignation by phone or in person.

Failure to comply with the regulations will result in blocking the internet registration!

Online registration regulations

  • Registering and failing to report for a visit may result in blocking the possibility of online registration.
  • Cancellation of the visit should be confirmed by phone.
  • Registration only for patients enrolled in our clinic.
  • Please enter your last name in the Login ID field.
  • Online registration possible on the day preceding the visit after 6.00 p.m. or on the day of the visit by 10.00 a.m.
  • Please provide a contact telephone number.
  • Please register with the doctor to whom you are designated or with the doctors whose names appear after selecting the option "ALL", except that adults go to internists, children only to paediatricians.
  • If there are no deadlines, please register by phone or in person.
