How to become a patient of our clinic?

To become a patient of the Health Clinic in Rumia, you must complete and sign the declaration of choosing a primary care physician. We also encourage you to choose a health visitor and community midwife (the choice of a community midwife is for women).

In order to submit a declaration, please go to registration at our Clinic.

The patient should have an identity document with a photo and PESEL number

  • ID card,
  • passport,
  • driving license.
  • children – school ID.

The declaration can also be submitted via the website office.gov.pl

You have 20 years and over, Are you insured by the National Health Fund?

Step 1

Please fill in the survey at your convenience:

  • Patient Internet Account (IKP) opens in a new tab, on the tab „Prevention”
  • mojeIKP mobile app, in the tab „Prevention”
  • directly at the primary healthcare clinic, which you are registered with, where you lodged your declaration of choice for your GP, nurse, and midwife.

Step 2

Tests will be commissioned based on the completed questionnaire:

  • The primary care practice issues a referral for tests
  • informs you when and where you should go to undergo your assigned tests
  • On the appointed day, report to the designated collection point.

IMPORTANT! The POZ clinic should contact you within 30 days.

Step 3

You are doing laboratory research.

The basic package for everyone includes:

  • blood morphology
  • Blood glucose concentration
  • Lipidogram
    • TC (Total Cholesterol) – cholesterol całkowity
    • LDL (Low-Density Lipoprotein) – LDL cholesterol fraction, the so-called „bad” cholesterol
    • HDL (High-Density Lipoprotein) – the HDL fraction of cholesterol, also known as „good” cholesterol
    • TG (Triglycerides) – triglycerides
  • creatinine level with estimated eGFR (kidney parameter)
  • TSH (thyroid-stimulating hormone)
  • General urine test.

You can also receive an additional package of tests depending on your age and the results of the health questionnaire (survey). The extended package includes:

  • Liver function tests (ALT, AST, GGT)
  • Total PSA in men over 50 years of age
  • anti-HCV
  • Lipoprotein A (performed once in a lifetime as part of a programme for those aged 20-40)
  • faecal occult blood test – FIT-OC method.

Step 4

You are registering for a summary visit at your GP surgery. You will be invited to it by your GP surgery once your laboratory results are ready.

The medical staff will check, among other things:

  • Pressure
  • Pulse
  • weight
  • Growth
  • waist circumference
  • hip circumference
  • BMI (Body Mass Index)
  • Waist-to-hip ratio (WHR).

Next, the medical staff will discuss the test results with you. If it is necessary, you will be referred for further diagnostics..

At your review appointment, you will receive Individual Health Plan, and in it recommendations regarding:

  • Nutrition
  • healthy lifestyle
  • physical activity
  • preventative health checks
  • and an individualised calendar of recommended vaccinations and educational advice.
