Safe Relationship Code – Version for Minors
You are at the NZOZ Panaceum Sp. z o. o. clinic in Rumi. We have various specialists working here, such as doctors, nurses, paramedics, and technicians. Everyone is here to help children in need.
Your being here means you need our help. We want you to feel safe and comfortable here. In the interest of your well-being and safety, we will be guided by the following principles in our daily work:
1. At the beginning, we will introduce ourselves to you. We will try to briefly explain what we do.
2. We would like you to tell us how we can address you. This is important to us.
3. We understand that everything happening in the clinic may be new and unfamiliar to you. We will try to explain everything.
4. You have the right to know about your health status and treatment plan. If anything causes you concern, please ask; we will always provide you with information appropriate to your age.
5. You have the right to express your views on medical and other matters, even if you are not yet 16 years old. Once you reach 16 years of age, you have the right to co-decide on your treatment.
6. We will try to give you choices where possible.
7. We want to be honest with you. If we know that a procedure may be painful, we will tell you.
8. Information about your health will be kept confidential and will not be disclosed to anyone other than you and your parents or guardians.
We will try to speak to you in simple language, so that everything is understandable, but if anything needs explaining, please tell us.
10. We would like someone else to be present during your examination, e.g. your mother, father, guardian, or another healthcare professional (e.g. a nurse). If you do not want your parent or guardian to be present during the examination, please tell us.
11. If you are frightened or worried about something, tell us about it. Where possible, we will try to find a solution or direct you to a place where you can get help.
12. You may also need support, such as a hug or comfort; please tell us about it. We can hug you only when you allow us to, we will not do it without your consent.
13. If you feel uncomfortable, unsafe, or worried about anything, please tell us. We will take action to ensure your safety.
Your parents or guardians may be with you during both the consultation and treatment room visits.
We want you to know that when you are at the clinic, you can bring your favourite toy, for example, a teddy bear, or a book. We just ask that you don't share them with others.
16. We want to be available for you, but there are times when we have to attend to other matters, so we can't always be with you. However, we will try to dedicate time to you once those matters are concluded.
17. During your stay at the clinic, with the consent of your parents/guardians, you may use a mobile phone or tablet, but only for safe content.
